This mushroom seems to have taken on some slug damage. Bright yellow cap.
Mixed woods (mostly deciduous) -- mostly maple.

Amanita species. When dealing with Amanita there are many look alikes even between species in this genus. It's best to get a knife and gently "dig" around and underneath the stem base and wiggle it free. This will give additional identifying features.
