Description:

The width of the cap in pic #4 was approximately 3.75 inches or 9.525 cm, and the over all height from base to cap was approximately 3 inches or 7.62 cm.

Habitat:

Found in a wood chip pile.

Notes:

The site I found the fungi on, said that it was edible. http://www.rogersmushrooms.com/gallery/D... The last 2 pics, were taken 2 days later.

I think this is a Lepiota sp. i would encline to say it's Lepiota Cristata, but I'm not sure.
