Description:

Cap ca 2.5 inches diameter; low convex. Gills white, close, free from stem. Stem ca.3 inches tall, 0.25 inches diameter. Spore print brown. Spores 6-8 x 4.5-6 µ; ellipsoid; smooth.

Habitat:

Growing on decaying hardwood log in deep shade on floodplain above a creek.
