Description:

Cap. Ca. 3 inches diameter, white with brown center; coarse scales. Gills. White, free. Stipe. Gray, ca. ¼ inches diameter and ca. 4 inches long; annulus present.

Habitat:

Solitary mushroom growing at the edge of a trail in a mixed hardwood/pine forest.
