Description:

Cap ca. 1.25 inches diameter; yellow center fading to cream at the margin; striations visible; white warts; distinctly slimy surface. Gills white; free; close with frequent short gills. Stipe ca 0.25 inches diameter at apex, slightly wider at base; fine scales; annulus remnants present; volva present. Spore print white

Habitat:

Lone mushroom. Growing alone in leaf litter on east-facing shore of a lake in a deciduous woods (predominantly oak).
