Hi Mark, thanks for the description. I think I might have found what you were looking for! Agrocybe molesta fits all the charactaristics you've point out. Here is a link to a description with some pictures.

Cap white turning pale yellow, smooth. Gills white crowded. Free from stem. Cap central. Remnants of veil shaped like teeth around cap. Veil around stem. Stem straight, and white and fibrous. Solid. No bulbous base.
