Amanita velatipes
Flat, creamy-yellow cap with white warts and a striate margin. White gills that were attached to the stipe. Gills were crowded with frequent short-gills. White stipe with an inverted funnel annulus. The stipe was shaggy below the ring, and smooth above it.
Growing on the ground in a deciduous forest.
