This turtle looks to have a shell deformity. It is important for box turtles to get natural sunlight in order to make the Vitamin D3 important in shell calcification. Here are a couple of links to proper care of this turtle:

If you cannot properly care for this turtle, please send it to a rescue and do not release it into the wild. It is unlikely to survive and could transmit disease that could cause native turtle populations to become ill. If you have questions, we will be quite happy to help you find the answers.
