Hygrocybe chlorophana or H. flavescens
Cap: 3/8"-1" wide, convex. Stalk: 1/4"-2", translucent at first then becoming whitish. Gills: white. Growing on decaying hardwood log.
Gully, finally concluded this is Hygrocybe flavescens as you suggested or H. Chlorophana. Different species names, but most likely the same. H. Chlorophana described 100 yrs earlier than flavescens. http://www.mushroomexpert.com/hygrocybe_...

I'm not sure how decayed a log needs to become before a fairly large Hygrocybe will grow on it, but this could be Hygrocybe flavescens.
