Galerina marginata
Largest cap, ca. 1.5 inches diameter. Gills attached (although they don't appear attached in the cross-sectional photos). Stem white at top, dark near base; hollow; tough; annulus present. No distinctive odor. KOH (3%); red-brown staining on cap. Spore print, rust brown. Spores subellipsoid, verrucose, 7-9µ x 4-5µ.
Growing alone, and in clusters on hardwood log.
Also known as Galerina autumnalis. Deadly poisonous.
