Striped Willow leaf beetle

Description:

Small beetle with black and white stripes. Has a head that is red in color. This beetle is small (perhaps 1/4 of an inch or so in size)

Habitat:

I did not find a lot of information on this beetle. I believe it can be found in mulch or on a host tree.

Notes:

I would like to know about this beetle, but had a difficult time locating information online. Any ref. links would be appreciated.
