Well the pointed eyes and classic mantis body indicate oxyopsis peruviana. The wings are too narrow to be pseudoxyops sp ( a similar genus.) and mantis religiosa does not live in peru. Oxyopsis peruviana is one of the most common mantids in peru. The spot on the wing mimics a flaw on a leaf. Yes a mantis ootheca(ooth for short) is an egg case of a praying mantis!
