Family name is just Curculionidae (no sp.!), Entiminae is the subfamily. Subspecies or ssp. would be the trinomial part of the scientific name. So correctly yours would be Entimine Weevil and Entiminae. In the scientific name field we only should use one name, in exceptional cases the subfamily or family (no sp. added).
