Thank you for all the comments. Yes, I think it is quite close to a hammer head planarian worm. And the pattern on the body is so similar to the one spotted by Harsha before. So cool! :)

Looks like some sort of land planarian/hammerhead worm. I wouldn't be sure of the species, unfortunately. http://en.wikipedia.org/wiki/Bipalium Here's the genus.
