Largish mushroom with dark stem with white scales below the ring, cap somewhat zonate and highly variable from flat to uplifted.
On well-decayed wood.
These pictures show the variability of the mushroom. All 3 pictures show what is likely the same organism (they tend to cover pretty large areas) with varying fruiting bodies.
