Coprinopsis atramentaria
A small group of common ink caps. These are older since they're starting to fall apart. This mushroom is toxic when consumed with alcohol, so it's also known as Tippler's Bane. Symptom severity varies with blood alcohol level.

Consuming this within a few hours of alcohol results in a "disulfiram syndrome" Symptoms include facial reddening, nausea, vomiting, malaise, agitation, palpitations and tingling in limbs, and arise five to ten minutes after consumption of alcohol.
