Description:

White mushroom with black scales. Cap, ca. 3-4 cm diameter; remnants of veil attached to margin of cap. Gills white; not attached to stem. Stem white; ca. 4mm thick; Ring present; white with gray-black margin.

Habitat:

Growing on old rotting log (species unknown) in mixed hardwood/pine (predominantly hardwood) forest. On north-facing slope above a lake.
