Description:

This mushroom was 7-8 cm tall and had a tan, convex cap with a few scattered beige warts. The gills were free, white, and crowded. The stem had a skirt-like ring and ended in basal bulb that was "chiseled" or split vertically in one or more places.

Habitat:

Growing on the ground in a mixed forest.
